I have a cousin who used to be Catholic, but is now Lutheran. He is still receiving Catholic communion, for instance when he attended my niece’s first communion. How does someone politely bring up his error without offending?

Just ask, I guess. Say something like “hey, I thought you were Lutheran” in a joking manner and work your way into what the Catholic Church teaches about communion.
