This is the one I know from grade school. It is similar to the ones above:

Oh my God, I am sorry for my sins. I have choosen to do wrong and failed to do good. I have sinned against you, whom I should love above all things. I firmy intend, with your help, to do penance, to sin no more, and to avoid whatever leads me to sin. I ask this in Jesus’ name. Amen
